Case Management Program
Our long-term community based Case Management Program encompasses identifying individual and family needs and planning for the now and the future. By identifying and coordinating community services to meet the needs and wants of the survivor, monitoring progress, re-evaluating the individual’s program and building support teams to assist the individual and their families, case management participants reach their goals. The case manager supports the individual as he/she plans for the future and moves toward living a life as independently as possible in their community. Goals are established by the individual and are based on their wants and needs; the case manager supports them in person-centered planning and through individualized service plans.

Eligibility:

A. Individual must have documented evidence of an acquired, non-degenerative brain injury caused after birth

B. Individual must be at least 18 years of age.

C. Individual must reside in Northern Virginia, including City of Alexandria, Arlington, Fairfax, Loudoun, and Prince William Counties.

D. Individual must sign a release for obtaining necessary assessment
information.

E. If the Case Manager believes that the individual has a problem with drugs and/or alcohol, he/she must participate in a substance abuse evaluation and must be willing to follow treatment recommendations.

F. If the Case Manager believes that the individual is a danger to him/herself or others, the individual must participate in a mental health evaluation and must be willing to follow treatment recommendations.

G. Legal Status - Must have legal status in the United States, including but not limited to, student visa, work authorization, Green Card.

Cost of the program:

Case Management services are provided at no cost to the individual. State and local governments, grants and donations provide funds for case management. There are a limited number of slots for services and admissions are based on a priority of need and least resources. There is a sliding scale fee for services the case manager may need to vend for on a case-by-case basis when there is no other resource to fill the service gap.

Referral:

Anyone can refer someone to the Case Management Program but the individual with a brain injury must desire and enroll in the program.
